Born, raised and educated in Oregon, Kim is a graduate of University of Oregon and Oregon Health and Science University. She likes taking care of whole families across life stages and generations. She enjoys helping patients find workable and fun ways to protect their own health while finding creative and often non-medical solutions to health problems. Kim enjoys learning from her patients as well. Most of all, she likes patients to have as relaxed and enjoyable a visit as possible; it can’t hurt if you laugh a little at an office visit. In the past she has volunteered as a Court Appointed Special Advocate for at-risk children, and a victim-offender mediator. She especially loves cross-cultural patient care, and is fluent in Spanish. Reading, music, word games and exploring Portland and the rest of the world are her after-work interests.

Illness or Injury Visit - New Patient

What do I get?

First time visit to our clinic to treat urgent or undiagnosed symptoms such as bronchitis / cough, earache / ear infection, flu-like symptoms, sinus infection / congestion, pink eye & styes, sore throat / strep throat, upper respiratory infection and other conditions.

What else does it include?

A thorough health history, allergy and medication history, review of old records as indicated and a single problem focused concern.

Exclusions/Prerequisites/Notes:

This visit is not intended to address
1. Life-threatening acute illnesses which are suited for the emergency department; or
2. Previously diagnosed chronic medical conditions such as diabetes, heart, lung, or kidney disease, or cancer.

Choose this service if you have not been seen at this clinic before.
NOTE: Pain medication cannot be prescribed at the first visit.

What do I get?

A return patient visit to our clinic to treat urgent or undiagnosed symptoms. This visit is intended for the treatment of urgent or undiagnosed symptoms of one system, such as bronchitis / cough, earache / ear infection, flu-like symptoms, sinus infection / congestion, pink eye & styes, sore throat / strep throat, upper respiratory infection and other conditions.

What else does it include?

This visit generally lasts 20- 30 minutes and includes system specific exam, diagnosis and treatment recommendations.

Exclusions/Prerequisites/Notes:

This visit is not intended to address
1. Life-threatening acute illnesses which are suited for the emergency department; or
2. Previously diagnosed chronic medical conditions such as diabetes, heart, lung, or kidney disease, or cancer.

Men's Wellness Exam

What do I get?

The Men's Wellness Visit, for the proactive management of overall health. It includes a holistic preventive health care history and physical exam.

What else does it include?

Blood pressure, height, weight, and BMI. Your care provider will examine eyes, ears, nose, mouth, thyroid gland, lungs, lymph nodes, heart, abdomen, reflexes and skin. Genital and prostate exam is performed for those 50 years old or older, or if requested.

Exclusions/Prerequisites/Notes:

  • Any problems that are noted will be referred to a follow up visit.
  • Other lab tests if recommended by your provider will incur additional costs.
  • Pain medication cannot be prescribed on first visit.

Sports Physical, Under Age 18

What do I get?

An exam targeting pre-participation physicals for sport activities. It will include a complete physical exam, assesment of joint health, medical health, and review of individualized nutrition in relation to fitness.

What else does it include?

Your provider will review new and old sports injuries and discuss treatment options.
This visit will include completion of any forms required by school or a sports league.

Women's Wellness Exam - Age 25 and Under

What do I get?

The Women's 25 and Under Wellness exam, primarily for patients who are between 18 and 25 years of age. It includes a thorough age appropriate history, lifestyle assessment and physical exam. Focus is on the needs of the older teen and young adult. A breast exam, pap smear and STD screens will be performed if requested.

What else does it include?

History taking and screens for possible health problems. Blood pressure, height, weight and BMI are measured. Your care provider will examine eyes, ears, nose, mouth, thyroid gland, lungs, lymph nodes, heart, breasts, abdomen, reflexes and skin.

Exclusions/Prerequisites/Notes:

  • Any problems that are noted will be referred to a follow up visit to be addressed.
  • Other lab tests, if recommended by your provider, will incur additional costs.
  • Pain medication cannot be prescribed at the first visit.

What do I get?

The Women's Wellness Visit, for patients between 26 and 64 years of age. This visit is for holistic health promotion. It will include a thorough health history, lifestyle assessment and identification of risk factors.

What else does it include?

Blood pressure, height and weight will be measured. Provider may examine eyes, ears, nose, mouth, thyroid gland, lungs, lymph nodes, heart, breasts, abdomen, reflexes and skin. A breast exam, pap smear and an HPV test will be included.

Exclusions/Prerequisites/Notes:

  • Any problems that are noted will be referred to a follow up visit to be addressed.
  • Other lab tests, if recommended by your provider, will incur additional costs.
  • Pain medication cannot be prescribed at the first visit.

Women's Wellness Exam - Age 65 and older

What do I get?

A wellness visit for women age 65 and older, this exam will cover all preventive measures necessary for optimum health.

What else does it include?

  • Detailed review of medical history and risk factors
  • Health screening including breast exam, pelvic exam
  • Heart and lung exam
  • Detailed discussion of diet and lifestyle

Exclusions/Prerequisites/Notes:

Screening for diabetes, high cholesterol, colon cancer and osteoporosis may be recommended and may include additional charges.
