A Nurse Practitioner (NP) in the State of Oregon holds a Masters or Doctoral degree in advanced practice Nursing; As an NP I provide independent high-quality healthcare services. The independence I have as a NP allows me to evaluate, diagnose, treat and follow-up on care for my patients. I order diagnostic testing and prescribe medications, refer for specialty consults as a part of my practice.

Since 1982 I have specialized in Women’s Health Care, covering the life span of health needs for women ranging from adolescence to elderly women. In 1998 I added the Family NP component, adding care for men and children. Now I diagnose and treat a wide range of health problems for all ages of people.

Besides clinical care my focus as a NP includes health promotion, disease prevention, health education and counseling. All these aspects of care help patients make wise health and lifestyle choices. My goal is to create a partnership in health with patients who choose to see me for their healthcare.

I participate in the care of cash paying patients as well as on the provider panels for Private, State and Federally funded health insurance plans. Also accepting new patients w/Medicaid and Medicare.

I bring to the practice PIH & SM a more traditional approach, one which could be defined as western medicine/healthcare. Over the years of practice as a NP and because of the Integrative component of our business I have developed a deep appreciation for the multi-dimensional aspects of needs and care for our patients.

Basic Urgent Care Visit

What do I get?

One visit for the immediate treatment of a minor injury or illness of a single symptom or diagnosis with ND, MD, OR FNP.

What else is included?

Depending on the injury or illness, may include evaluation and treatment of urgent injuries or illnesses such as allergies, colds, contact dermatitis, cough, congestion, cuts and wounds, fever, flu, infections, insect bites, minor burns, nausea, pink eye, rashes, sprains and strains, strip, sore throat, or urinary tract infections.

Exclusions and Notes:

This visit is not intended to address

  1. Life-threatening acute illnesses which are suited for the emergency department; or

  2. Previously diagnosed chronic medical conditions such as diabetes, heart, lung, or kidney disease, or cancer requiring procurement and review of previous visit notes from another provider office or hospital.

  3. The pricing is for the visit alone and does not include any additional testing, services, supplies and/or procedures that may be required at the time of appointment and will incur additional costs. The provider will discuss options and costs with patient, and obtain consent for any services that incur additional expense to the patient.

  4. Appointment Duration Note: an average visit lasts between 15 and 30 minutes.

DOT Physicals

What do I get?

One Physical Examination for Department of Transportation Certification.

What is a DOT Physical Exam?

This is a periodic physical examination to obtain Medical Examiners Certificate (DOT Card).

•DOT card may be issued for up to 2 years
•Please fill out paperwork prior to attending exam

IUD Insertion

What do I get?

The IUD insertion consists of two visits. The first visit will include a thorough discussion of this form of contraception, choices involved, contraindications, and possible complication. The second visit will include the IUD insertion procedure.

What else does it include?

A pelvic exam will be performed. Eligible candidates will return to clinic for the IUD insertion.

What is an IUD?

An IUD, or intrauterine device, is a form of birth control. It is an object placed in the uterus to prevent pregnancy. Among modern IUDs, the two types available are copper-containing devices, and a hormone-containing device that releases a progestogen. Please speak with a health care provider for more information.

IMPORTANT NOTES:

  • The IUD device is ordered through the clinic and incurs an additional charge. Please contact the clinic directly for pricing information.
  • The appointment time booked is for the first visit only. The first visit is a consultation and is considered free of charge. Payment of this service at the time of booking is for the IUD insertion procedure. If, during the consultation, you decide against going forward with the procedure, please notify the clinic and they will arrange for a full refund. Refunds take approximately 5 business days to process.

IUD Removal

What do I get?

This visit is appropriate for either the new or return patient who desires to have her IUD removed.

What else does it include?

This exam will include consultation and warnings, pelvic exam and removal of the IUD.

Exclusions:

This visit is not intended for contraceptive counseling, nor life-threatening complications which are suited for the emergency department.

Men’s Wellness Exam

What do I get?

The Men's Wellness Visit is inteded for the proactive management of overall health including a basic preventive care history and exam.

What else does it include?

This exam screens for possible health problems. Blood pressure height and weight are measured. Your care provider likely will examine eyes, ears, nose, mouth, thyroid gland, lungs, lymph nodes, heart, abdomen, reflexes and skin.

Exclusions/Prerequisites/Notes:

  1. Any problems that are noted may need referral to another care provider.
  2. Other lab tests if requested by the provider will incur additional costs.

Sports Physical Exam Age 18 and Under

What do I get?

A sports physical exam which targets pre-participation physicals for sport activities.

What else does it include?

Exam includes a complete physical exam, assessment of joint health, medical health, and review of individualized nutrition in relation to fitness. Your provider will review new and old sports injuries and discuss treatment options.

Exclusions/Prerequisites/Notes:

This visit will include completion of any forms required by school or a sports league.

What do I get?

A new patient office visit for the treatment of an urgent injury or illness of undiagnosed symptoms such as bronchitis / cough, earache / ear infection, flu-like symptoms, sinus infection / congestion, pink eye & styes, sore throat / strep throat, upper respiratory infection and other conditions.

What else does it include?

Depending on the injury or illness, this visit may include the routine treatment of cough, congestion, ear pain, fever, sore throat, rash, UTI, suture removal and acute sprains/strains.

Exclusions/Prerequisites/Notes:

This visit is not intended to address
1. Life-threatening acute illnesses which are suited for the emergency department; or
2. Previously diagnosed chronic medical conditions such as diabetes, heart, lung, or kidney disease, or cancer.
